Each student must submit a completed enrollment packet which includes the following forms:
Student Record Checklist
Enrollment Application and Agreement Form
Student Medical Form
Registration, Tuition and Refund Policies
Tuition Express Debit Card Form
Land’s End Instructions
Children’s Get to Know Me (Children Enrolled in 18 months and 2’s Only)
Confidential Form
In addition to these forms, please include a copy of the most Updated Immunization Record (signed by the doctor) in the packet.
These forms must be filled out accurately and in full. A non-refundable annual registration fee, supply fee, and payment for the last month’s tuition are due upon registration to secure your child’s place in the program.
Once you have completed all forms, please print them and bring them to our office.
